2009 Re-Application Information for the Critical Use Exemption of Methyl Bromide

INSTRUCTIONS

For those applicants that are re-applying for a Critical Use Exemption (CUE) for 2011 and 2012 the U.S. Environmental Protection Agency may already have some of the needed information. This document lists the portions of the CUE application form which will not have to be completed unless the information has changed. For those applicants that completed an application form in 2002 through 2007 but did not complete an application in 2008, the entire form must be filled out.

As you may know, the U.S. government uses the information you submit in the application to develop the U.S. Nomination for a CUE for Methyl Bromide. Applicants may also wish to review the Nomination submitted to the Ozone Secretariat in 2009 to see if any information should be augmented.

Complete only those portions of the worksheet that have changed.

Complete all portions of the worksheet.

Worksheet 2D (1&2), Methyl Bromide – Baseline – Operating Costs for 2008
Complete all portions of the worksheet.

Worksheet 3A, Alternatives – Technical Feasibility of Alternative to Methyl Bromide
Complete only those portions of the worksheet that have changed. Only include research data newer than 2002 unless not previously included. Include relevant studies newer than 2002 even when the results are not different than previously submitted research.

Worksheet 3B, Alternatives – Changes in Operating Costs
Complete only those portions of the worksheet that have changed.

Worksheet 4, Emission Control
Complete all portions of the worksheet.

Worksheet 5, Future Research Plans
Complete all portions of the worksheet.

Worksheet 6, Application Summary
Complete all portions of the worksheet. Please include the pounds and area requested for 2011 and 2012.
