APTI Registration Procedures and Forms

All self-instructional courses are available on-line in PDF format If you do not need to receive CEU credits for a course, you can study the course material, take a test and upon passing the test, you can print a certificate at your desktop. For this, no registration with APTI is needed.

If you would like to receive CEU credit for a course, use the table below to determine if you are eligible and to understand how CEUs are credited.

For Classroom Courses

• Registration procedures vary among sponsoring organizations and course providers. It is the responsibility of each student to contact the sponsoring organization or course provider to confirm registration procedure. • Consult the APTI Training Calendar for specific course dates and times.

APTI Forms

Application for Training Use this form to register with APTI and to obtain a PIN. The PIN is a self assigned number (max 10 digits). If the PIN you choose is already used you will be notified. This form should only be completed one time.

Information Change Request Use this form to update information such as name, address, phone etc with APTI.

Class Registration
This form does not need to be completed for self instructional courses that are taken online.

Request for Transcript Print, complete, sign, and fax or mail to the address listed below. A copy of the APTI courses completed will be sent to you.
