proud to partner with the US Environmental Protection Agency to support
communities across the country as we all work together to tackle asthma. If

Hundreds of providers, health plans, health departments, community asthma coalitions, researchers, policy makers, and other leaders gather to discuss the most effective community-based strategies for improving asthma program outcomes, building successful and sustainable asthma care programs, and extending the reach and impact of great programs to deliver high-quality asthma care to everyone in need.
The Forum provides attendees the opportunity to discover best practices, explore the latest science, hear from leading asthma care programs, and develop leadership and action plans for:

- Providing effective clinical care across your community.
- Delivering tailored environmental interventions that make an impact.

About the National Forum
The Communities in Action for Asthma-Friendly Environments National Forum provides a venue for dedicated asthma care leaders to learn from one another and from some of the highest performing asthma programs in the world. The Forum explores successful approaches to asthma care with leading programs, scientists, policy makers, and others; enables participants to benchmark their programs against the best national strategies for improving outcomes and sustaining their programs; and provides the access to the strategies, resources, tools and community of support to help program leaders to transform the delivery, impact, and scale of asthma care in their communities. What brings us together is our commitment to transforming the quality, scope and reach of community asthma care to ensure that everyone who needs it receives the best possible care in every community in the U.S.

Who Should Attend?
Representatives from community-based asthma programs, coalition members, policy makers, and others should attend the National Forum. Past attendees have included representatives from health care plans and providers, government agencies, advocacy and other nonprofit organizations, schools, employers, universities, and others. Communities will realize the greatest benefit if multiple people from various programs that comprise a community-based asthma care system attend, because the Forum is designed to facilitate team-based learning and action planning.
