Highlights

Purpose of the Survey

In an effort to further develop, implement, and improve the SC3 program, EPA will be conducting an annual voluntary survey of the SC3 Partners to learn about their experiences and needs under the SC3 program.  The survey is divided into three sections:

Responses to the survey will enable EPA to evaluate and improve SC3, as well as share information about the program with others. Data gathered through the survey will not be used for modeling or statistical analysis.

EPA has created two survey forms for partners to download for their use:

  1. Initial Survey (Word) (21 pp, 375K, Download the free Microsoft Word Viewer)—This survey form should be completed by partners who are participating in the SC3 survey for the first time. It is designed to give the SC3 program a general idea of a partner's background, accomplishments, and needs under SC3.

  2. Annual Update (Word) (21 pp, 368K, Download the free Microsoft Word Viewer)—Partners should complete an Annual Update in each subsequent year of their partnership. This survey is designed to describe a partner's accomplishments and needs since the previous survey.

Completed surveys should be submitted to EPA no later than September 30th of each year.

EPA developed this spreadsheet (Excel) (1 pg, 40K, Download the free Microsoft Excel Viewer) to assist partners in data entry for worksheet 4 of the survey. The spreadsheet includes entries for chemical name, waste quantity, and chemical properties. Please limit one school per spreadsheet page.
