Forms and Information for Applying for and Managing Your Assistance Agreement

The information and documents here are applicable to any GLNPO grant, though they were originally developed to assist applicants submitting federal applications following their selection through GLNPO's annual Funding Guidance process. Of particular interest to Applicants applying for a grant:

• Item HI3 is a checklist of the 11 items an applicant should submit to formally apply for a GLNPO grant
• Item HI4 provides supplemental Instructions and Requirements keyed to the Checklist, including the address to submit applications and ways to avoid common causes of delays.

• These are Adobe Acrobat files.
• The Adobe Acrobat files can be opened by simply clicking on the name below if you have either the free-version Acrobat Reader or the full-version Acrobat.

• Many of the Adobe Acrobat Forms are of the fill-in type. They can be filled-in and printed using either the free-version of Acrobat Reader or the full-version of Acrobat. However, only users of the full-version of Adobe Acrobat will be able to save these filled-in files. those using the free-version of Acrobat Reader, the filled-in information will be lost as soon as the file is closed or the computer is shut off.
  Of course, you can also print out the forms and fill them in on a typewriter or neatly print the information on them.
• GLNPO will update links on its website for each of these documents whenever GLNPO updates its own documents, or is aware of updates by others.

           application.
• A2.   SF-424. Application for Federal Assistance
• A3.   SF-424A. Budget Information [fill in]; SF424A. Instruction (PDF 56Kb, 2 pages)
• A4.   SF-424B. Assurances - Non-Construction Programs  [fill-in]
• A5.   Workplan Format  (Item 5 on page 3 of HI4).
          Do include milestones and a budget chart
• A6.   Itemized Object Class (Item 6 on Page 9 of HI4).
          Category Budget Narrative. A particular format is not required
• A7.   SF LLL-A.  Certification Regarding Lobbying
• A8.   SF LLL.  Disclosure of Lobbying Activities  [fill-in]

• M14.  Great Lakes Restoration Initiative Logo · Appropriate signage at "on-the-ground" protection or restoration projects under the Great Lakes Restoration Initiative must give project information and credit the initiative and appropriate federal agencies for funding. Signs should clearly display the Great Lakes Restoration Initiative logo.
