The briefest description possible of how to successfully apply for an equivalency recommendation is to notify the Pathogen Equivalency Committee of your intent, develop a sample and analysis plan to gather data in support of your process's equivalency, carry out that sampling plan, then complete and submit an official pathogen reduction equivalency application package (see link below). The Pathogen Equivalency Committee will then evaluate the package and make its recommendation through the appropriate channels. The Equivalency Recommendation Process. 1) Initiation Phase: Contact Permitting Authority; Contact PEC. 2) Quality Assurance Project Plan (QAPP) Phase: Prepare QAPP; PEC Reviews QAPP; Implement QAPP. 3) Application Phase: Prepare Application; PEC Reviews; Prepare O&M Manual 4) Notification Phase: PEC Issues recommendation to the Office of Science and Technology (OST); OST Reviews; OST Forwards Recommendation to Permitting Authority. Finally, the Permitting Authority may issue Formal Equivalency.

Quality Assurance Project Plan A QAPP ensures that the desired quality in sample collection, laboratory analysis, data validation & reporting, and documentation & record-keeping are achieved and maintained.		Completeness Checklist (DOC) (8 pp, 263 KB) This checklist is used by the PEC to review submitted QAPPs and equivalency applications. It is provided to help applicants double-check that all required and applicable elements have been addressed in their QAPP/equivalency application before submittal.
Equivalency Application Package (DOC) (7 pp, 158 KB) This is a ready-made fill-in form including instructions for submitting an official pathogen reduction equivalency application package.
