Download Forms II Lite
The FORMS II Lite software is available at no cost for use by or for any federal agency. By downloading and installing FORMS II Lite, you agree to the following license agreement:

This software is being provided compliments of the U.S. Environmental Protection Agency. Please note that the use of this software provided under license is strictly limited to activities supporting U.S. government initiatives.
FORMS II Lite 5.1 was developed by CSC for the USEPA under GSA Contract GS-35F-4694G.
FORMS II Lite is a trademark of USEPA.
Download FORMS II Lite Version 5.1 (ZIP, 36 MB) - To install the FORMS II Lite software, unzip the provided installation files to a temporary directory and run setup.exe. A license number is required for installation, and may be obtained from the FORMS II Lite Help Desk at 703-818-4200 or via email at f2lite@fedcsc.com. FORMS II Lite Help Desk assistance is available Mondays through Fridays from 9AM to 5PM (Eastern Time). Please contact the Help Desk if you have any questions or need assistance with your installation.
Information for Microsoft Windows 7 and Vista Users
To use FORMS II Lite on the Microsoft Windows 7 or Vista operating systems, users must apply some compatibility features. These Windows 7 and Windows Vista compatibility features must be applied using the computer’s administrator account. To make FORMS II Lite compatible, complete the following actions after installing the software:
- From the desktop, right click on the FORMS II Lite icon and select the ‘Run as Administrator’ option.
- From the desktop, right click on the FORMS II Lite icon and select the ‘Properties’ option. Under the ‘Compatibility’ tab, select the ‘Run this program in the compatibility mode for’ option and choose ‘Windows XP’ from the drop down box. Click the ‘Apply’ and’ OK’ buttons to save the changes.
After these actions have been completed, FORMS II Lite should be compatible in Windows 7 and Vista. For more information regarding FORMS II Lite’s compatibility, please contact the FORMS II Lite Help Desk at 703-818-4200 or via email at f2lite@fedcsc.com.
Upgrading from Earlier FORMS II Lite Version 5.1 Releases
Users who received the FORMS II Lite Version 5.1 software prior to the 5.1.47 release will need to install the most recent patch in order to utilize all of the new features. Please consult the FORMS II Lite Version 5.1.47 Release Notes (PDF) (1p, 77K) for the detailed list of changes in version 5.1.47.

To install the Upgrade:
- Make sure that the FORMS II Lite application is closed.
- If you are currently using FORMS II Lite Version 5.1 (5.1.43), download both the Upgrade and the Grid files provided above. If you are currently using FORMS II Lite Version 5.1 (5.1.45 or 5.1.46), download only the Upgrade file provided above.
- From the .zip file, extract all of the files into the FORMS II Lite program file.
After the upgrade has been installed, users may import the laboratory information reference table (provided below), which contains all of the current Contract Laboratory Program (CLP) laboratories.

This laboratory information reference table provides users with a file containing current information for CLP laboratories that can be imported into FORMS II Lite.

Import Instructions
- Save the Lab.F2R file to the desktop of your computer.
- Open the FORMS II Lite application.
- Close the FORMS II Lite dialog box if it appears when the program is opened.
- From the Main Menu Bar select File, then select Import/Export to open the Import/Export Wizard.
- Select Import Reference Tables, and then click the [Next>] button.
- In the Import Reference Table window, enter the name of the file or click the [Browse] button to select the (.F2R) file to be imported, and click the [Finish] button.
- Select the Update Existing Information option to add any new records and to make any changes that appear in the import table, to the data in the original reference table.
- Click the [Finish] button to complete the import.
Utilities File for Troubleshooting
This file contains tools which can be used to resolve various issues. To use this file unzip it using the file compression software of your choice and follow the instructions below.

Users should run the mdac_typ.exe file when any of the following error messages are received:
- “Error 1919: error configuring ODBC data source FORMS2Lite51.”
- “Run-time error 3704: Operation not allowed when the object is closed.”
Users should run the FixUpOffice.exe file when any of the following error messages are received:
- "Load Library Error" for the .ocx files.
- "Runtime error 440: Automation error."
- "Runtime Error 339. Component FM20.dll or one of its dependencies is not correctly registered. The file is missing or invalid."
- “Runtime error 339 ‘component’ Crystal32.ocx or one of its dependencies is not correctly registered; a file is missing or invalid.”
Running the FixUpOffice.exe file:
- Ensure that FORMS II Lite is closed.
- Open the Utilities folder and run the FixUpOffice.exe file.
- Accept the terms and conditions of the license agreement if prompted.
- Select “Continue” once the welcome module appears.
- Click the computer icon on the left hand side of the module to begin the installation of the Microsoft ActiveX Control Pad.
- Locate the “Microsoft ActiveX Control Pad” item from the list of program groups that appears and select “Continue."
- After the installation of the Microsoft ActiveX Control Pad has been completed successfully, select “OK.”
- Restart the computer if prompted to do so.
