Soil Fumigant Management Plans

Soil fumigant labels require users to prepare a site-specific fumigation management plan (FMP) before the application begins. For additional information please see EPA’s fact sheet, Phase 2 Site-Specific Fumigant Management Plans and Post-Application Summaries (PDF). (5 pp, 252k)
Templates have been developed that fulfill the elements required by the labels. Please note that use of these templates is not required, and users may develop their own FMP to meet the label requirements.

EPA Phase 2 Templates
EPA created the templates in PDF and Word formats for the different soil fumigants. Users can choose which format they prefer. The PDF templates are for users to print and fill out by hand and the Word templates are for users to fill out on the computer. The agency also created samples of filled out FMPs, which also include sample filled out Post-Application Summary reports and samples of filled out related forms for each chemical. Since label requirements vary based on the soil fumigant, please use the appropriate chemical-specific templates.
