All grantees are strongly encouraged to submit Property Profile forms via ACRES (Assessment, Cleanup, and Redevelopment Exchange System)

If you are unable to submit the Property Profile form via ACRES, you may fill out, print, and submit the following form to your PO:

- MBE/WBE Reports (under old MBE/WBE Program for grants awarded prior to May 2008)

- Non-Profit Entities (PART 30 Recipients) due on an annual basis by October 30th of each year

Closeout reports must be submitted no later than 90 days after the end of the cooperative period of performance. All forms except Progress Reports and Property Profile forms should be submitted to Kathleen Blinebury. The following must be submitted:

- Final DBE Reports (for grants awarded after May 2008)
- Final Lobbying and Litigation Certification
- Final Progress Report
