After you have returned one signed copy of your grant agreement to EPA Region 9, you may request payment for grant-related expenses as needed, using the form below.

2. Submit Final Federal Financial Report (FFR) form SF 425 to the Las Vegas Finance Center

A final Financial Status Report is due within 90 days of the expiration of your grant. You should refer to the regulations and terms and conditions included with your grant for this requirement.

The FFR form is available (XLS). You can fill in the form and email from your browser if your computer is configured to do so. If not, see the alternative procedure in the box at right. Note: Instructions for completing the form are included.

  1. Complete the form using one of these methods:
    • Print it and complete it manually (printing neatly or using a typewriter).
  2. Mail or fax to the Las Vegas Financial Center mailing address.

3. Submit Interim Federal Financial Report (FFR) form SF 425 to the Las Vegas Finance Center

Using the Adobe PDF interactive documents – Requires Adobe Reader version 5 or higher (download from here About PDF). These forms were designed for you to fill in and print from your browser. However, if your computer is not set up to do this successfully you can use this alternative procedure:

  1. Open the form from this page by clicking its link.
  2. Save the form to your local computer.
  3. Using your Adobe Reader, open the form you just saved.
  4. Fill in the form as needed.
  5. Use the Save As... feature of the Adobe Reader to save the filled in form to your computer. (This preserves the original for another use.)
  6. Print the completed form, then FAX or mail it to the address indicated on this page.

4. Submit Minority/Women-Owned Business form EPA 5700-52A to EPA Region 9 Grants Office

Minority Business Enterprises/Women-owned Business Enterprises Utilization Report (PDF) (5 pp, 80K) (EPA 5700-52A)
Unless otherwise specified in the grant award, this report must be submitted to the Grants Management Office by October 30 of each year. Refer to the regulations and terms and conditions of your grant for this requirement.

  1. Complete the form using one of these methods:
    • Print it and complete it manually (printing neatly or using a typewriter).
    • Complete it online. Save to your local computer and print it. (See the instructions in the box above marked "Using the Adobe PDF.")

To ensure that your organization/agency is complying with the appropriate MBE/WBE or DBE Program requirements when procuring Services, Supplies, Equipment, and Construction with EPA funds and any matching dollars, please note the award date as well as the terms and conditions of your organization/agency's financial assistance agreement/grant with EPA.

5. Submit Lobbying and Litigation form EPA 5700-53 to EPA Region 9 Grants Office

Lobbying and Litigation Certificate (PDF) (1 pg, 30K) (EPA 5700-53)
This certification must be signed by the head of your organization and submitted to the Grants Management Office at the EPA Region 9 address within 90 days after the end of the project period.

    • Complete it online. Save to your local computer and print it. (See the instructions in the box to the right.)
  2. Mail to the EPA Region 9 mailing address.

6. Submit Progress and Technical Reports to EPA Division Project Officer

Generally, your work plan for the project and/or the terms and conditions included on your grant will specify the frequency and types of progress/technical reports that are required. If there are no specific requirements listed, you must submit a final performance report to the EPA Project Officer within 90 calendar days after project completion. 40 CFR Part 30 and 31 identify what this report must include.
