The use of FORMS II Lite and/or Scribe is mandatory for all Contract Laboratory Program (CLP) samples. For detailed instructions on how to upload XML files to the Electronic Sample Documentation System (ESDS), download the instructions available in the ESDS section of the ASB Tools page. See the following directives for details on field sampling software requirements and exemptions:
- Directive 9420.0-38 (PDF) (4 pp, 243K) - This is the original directive requiring the use of FORMS II Lite for CLP samples.
- Directive 9200.1-103 (PDF) (3pp, 2.8M) - This directive permits the use of Scribe software for Superfund CLP and CLP-NRAS sampling activities.
General Information
The FORMS II Lite software is a flexible and easy-to-use, stand-alone, Windows-based application that simplifies and accelerates the sample documentation process. FORMS II Lite reduces the generation of hand written documents by nearly 70 percent. Specifically, FORMS II Lite:
- Generates sample labels, bottle tags, and Chain of Custody (COC) forms;
- Tracks samples from the field to the laboratory;
- Facilitates electronic capture of sample information into databases; and
- Exports data electronically as .xml, .dbf, or .txt files.
Minimum System Requirements
Recommended minimum configuration requirements for the user's system are as follows:
- Windows 98, NT, ME, 2000, or XP;
- Microsoft Internet Explorer 4.01 or higher;
- 200 MHz Pentium processor;
- 64MB RAM;
- 35MB available hard disk space;
- 2X CD-ROM drive;
- 800x600 resolution using small fonts; and
- Mouse interface, printer, and remote power sources for field use.
